You grip it close to where the keys are, then use the part sticking out of the bottom of your fist to strike vulnerable areas and it can also be used in holds like a wrist lock.
Trying to find the actual name but I think it’s one of those self defense sticks.

From all the ER footage I’ve seen, everything can be dual use if you’re brave enough.
