This is actually explained within BG3 through books and a sidequest (as well as the actual PnP source material). Basically the town was started by a dude named Balduran. At first it was just a small dock, being one of the only places along the Sword Coast you could safely make port since the coast is mostly huge sheer cliffs or rocky shallows. Eventually because of its strategic location it grew, and for safety gates were built. Starting with 1 but eventually expanding to 3 (this is not a joke to go along with the title; they really do have 3 major gates going from the inner to outer city and the docks).
Balduran himself was a legendary explorer even before settling down. Though he did eventually leave Baldur’s Gate and set sail West, across the ocean and was never heard of or seen again.

There’s a weird culture in the city where you don’t just toss around his actual name willy-nilly. It’s why citizens of the city are called Baldurians instead of Baldurans. Balduran himself didn’t name the city, IIRC. It was originally just “The Gate.”
As for a 4th gate: Hey, they can always build more gates. 😁
